No reservation made by phone will be held for more than 48 hours without a deposit.
Deposits are non-refundable and non-transferable.
Complete paperwork as indicated on confirmation letter.
Paperwork must be returned by date indicated on confirmation or your camper will not be allowed to attend.
If your camper has not attended camp within the past year, call ext. 111 and ask what forms are needed to update file, then follow above instructions.
DDS -Spaces for campers registering with a valid vendor service authorization from DDS must be approved by Horizons and are limited.
At least 4 weeks prior to the weekend or summer session selected, send in the following paperwork completed:

White Permanent Health History Form
White Health History Update Form with a copy of Health Insurance card
White Photo/Video Release Form with a photo
Parent/Guardian Skills Check Sheet
Teacher/Supervisor Skills Check Sheet

You will receive a phone call once ALL paperwork has been submitted to complete the approval process.
Do not send any money until your camper is approved. Your check will be returned to you.
No space will be held until all paperwork is submitted.

First time campers may not register for New Years Eve.
